Injectors Failing?

I'm not really sure what causes this:

On random occasions (at least a couple times a day but at no specific moment) my injectors will fail. On my PowerFC comander I watch an injector duty will go to 0.0%. This occurs when I am between 0~50% throttle. All of the sudden it the injectors will just cut out as if I'm not even on the gas... Also when I am at idle if I hold my foot on the gas peddle it will raise to abou 1500 ~2000 rpm then drop back to 500rpm repedatively. It just keeps doing it. I don't think it's a vacuum leak because it only does it when my foot is on the gas pedal...

Could this be a Failing MAF?

I am stumped. this has been going on for weeks and I still can't get any conclusions..

reseat your ecu and pfc connections.........

do you drop cylinders when this happens? engine shut off?

Ive had mafs problems where the car would buck and die, id smack the mafs and it would come back........ possibly mafs. no airflow signal = no pulse width.
